Do guys care about what they look like with girls? Why did he care?

I always thought that guys for the most part don't care about how they look around girls, except for maybe when around a girl they like. I went over to bring this guy something he had asked me to and when I get there, the first thing he says or kinda like apologizes for how he looks, cuz he was in sweatpants and stuff and was kinda sick. He seemed to not want to look me in the eye or just overall seemed less self-confident than usual ( okay, a lot self confident). Why did he care about that and is that normal? As far as what we are, I'd say we're new friends who've messed around a bit and hey, i wasn't looking my best eiither.
